Job Description

Position Summary We are seeking a detail-oriented and organized Medical Records Clerk to join our team at a skilled nursing facility. This role is responsible for maintaining accurate resident medical records, ensuring compliance with regulatory standards, and supporting clinical staff with health information management. Key Responsibilities Maintain and organize resident medical records in accordance with federal, state, and facility regulations Assemble, review, and file medical documents in electronic and/or paper charts Ensure accuracy, completeness, and confidentiality of all records Process physician orders, admissions, discharges, and transfers documentation Scan, upload, and index records into the electronic medical records (EMR) system Respond to requests for medical information in compliance with HIPAA Assist with audits, surveys, and compliance reviews Coordinate record retention and storage procedures Support nursing and administrative staff with documentation needs Qualifications High school diploma or equivalent required Previous experience in medical records, health information management, or healthcare setting preferred Experience in a skilled nursing or long-term care environment is a plus Knowledge of HIPAA and medical terminology preferred Proficiency with electronic medical records systems and Microsoft Office Strong organizational skills and attention to detail Ability to handle sensitive information with confidentiality Excellent communication and teamwork skills Benefits Competitive pay Health, dental, and vision insurance Paid time off and holidays 401(k) retirement plan Opportunities for growth and advancement About Us We are a dedicated skilled nursing and rehabilitation center committed to providing compassionate, high-quality care to our residents.
