Our primary goal is to foster physician collaboration to deliver the most advanced pediatric care throughout Northern California and beyond. We have Pediatric, Ophthalmology, Adult Medicine, and Women's health clinics located throughout the San Francisco Bay area.
Position Summary:
Responsible for maintaining accurate and complete patient medical records across the organization. This includes the processing, scanning, indexing, and management of health information documentation in the electronic health record (EHR). The Medical Records Clerk ensures records are properly handled, easily retrievable, and compliant with privacy and regulatory standards. Works collaboratively with clinical and administrative teams to support continuity of care and operational efficiency.
Responsibilities:
Daily scanning and indexing of incoming medical records and documentation into the electronic health record (EHR) with strong attention to detail. Ensure all patient records are complete, accurate, and properly filed in accordance with established procedures. Process incoming and outgoing medical records requests, ensuring compliance with HIPAA and organizational policies. Maintain and update patient demographic and record information as needed for accuracy. Respond to internal and external inquiries regarding medical records in a timely and professional manner. Monitor work queues and ensure timely processing of all documentation. Perform document quality checks to confirm legibility, accuracy, and proper categorization. Maintain confidentiality and security of all patient health information at all times. Assist with record retention, archival, and destruction processes in accordance with policy. All other duties as assigned.

Critical and Analytical Thinking:
Ability to review records for completeness and resolve discrepancies.
Technical Skills:
Strong computer skills, including use of Electronic Health Records (e.g., EPIC), scanning systems, and Microsoft Office tools. Communication (Oral and Written): Effective verbal and written communication skills. Ability to communicate professionally and maintain confidentiality.
Professional Accountability:
Demonstrates teamwork and maintains positive working relationships. Ensures work meets quality, regulatory, and timeliness standards.
Quality and Service:
Maintains high standards for accuracy, compliance, and service. Supports EHR data integrity and optimization. Identifies and reports potential compliance or quality concerns.
Other Job Functions:
Performs other duties as assigned.
Skills:
Computer proficiency (Internet, Excel, Word, Outlook, EPIC or other EHR systems).
Ability:
Ability to multi-task and work independently. Strong attention to detail and organizational skills. Effective verbal and written communication skills.
Education:
High School Diploma or GED required.
Knowledge:
Medical terminology. Health information management practices and HIPAA regulations. Document handling, scanning, faxing, and record management processes.
Experience:
1+ years of experience in healthcare, health information management, or medical office setting preferred.
Physical Requirements :
Must be able to lift up to 20 pounds. Must be able to sit, stand, or walk for extended periods of time.

Maintains communication with managers and colleagues on workflow and issues. Builds effective working relationships across departments. Demonstrates respect for cultural diversity, patient rights, confidentiality, and teamwork.
