Job Description
Position Summary The Medical Records Specialist prepares daily charts for patients seen within the Advanced Thoracic Oncology Center. Requests and obtains any documentation needed prior to clinic visit, including but not limited to medical history, diagnostic images, and lab tests. Ensures that patient testing information is placed in patient's chart for provider review. Works closely with the providers and clinical team to assure data integrity and performs other clerical duties as assigned. Education None Required Experience 3-5 Years Medical Office and Medical Records Management - Required License & Certification None Required Core Job Functions Assembles patient health information for each clinic visit, including but not limited to, medical history, exam results, X-ray reports, lab tests, diagnoses and treatment plans. Verifies documents are completed accurately, indexed correctly and/or properly identified, and signed. Ensures all necessary information is located in electronic chart (or paper chart) in proper chronological order. Retrieves imaging for all radiology tests; communicate with physician or office manager when not available. Identify report availability for physician review and signature. Ensures all orders have been completed and results received in advance of appointment and/or communicates with clinical specialists and Practice Coordinators in advance if otherwise. Assist in preparation, download, and routing of overnight oximetry reports, home sleep study data, and CPAP equipment data for provider review. Prepares records accurately to ensure timely access by authorized persons. Monitors chart for additional work assignments. Organizes and stores records appropriately to establish easy access for clinical personnel. Phones are answered in a timely and courteous manner. Accurate information is provided, taken from caller and delivered to the appropriate recipient. Adheres to the release of patient information and is compliant with HIPAA regulations. Releases protected health information in an efficient manner to appropriate parties.
