Job Description

Primary function. Compiles, scans, processes, and maintains medical records consistent with medical, administrative, ethical, legal, and regulatory requirements. Utilizes electronic health record system for inquiry purposes, enters and sends data, and generates reports. Releases medical information for clinics as directed. Birth and Fetal Death certificate submission. Cancer Registry submission. All other duties as assigned by department manager. Duties and responsibilities Organize reports for scanning to patient clinic accounts. Determine and sort if reports originated at CH-F or from an outside source. Scan Documents to correct EMR location. Assists in maintaining the integrity of the medical record by organizing and scanning documents to the proper location for ease of accessibility and complete medical records. Scan documents created by CH-F and acknowledged by provider to a separate folder in CH-F network Maintains continuity of work by documenting and communicating actions, irregularities and continuing needs. Performs all other duties as assigned. Nothing in this specification restricts management's rights to assign or reassign duties and responsibilities to this job at any time. Biller duties as assigned by supervisor of billing. Admissions backup as needed for both hospital and clinic. This may include weekends, holidays or other days as needed.
Qualifications Qualifications include:
High School Diploma or GED Equivalent. Medical dictation/terminology experience preferred Basic Knowledge of Computer, Microsoft Outlook, Word, and Excel, operation of fax, copier, and scanner. Excellent communication and interpersonal skills. Time Management/Organizational skills. Attention to detail/quality focus. Productive/deadline oriented. Professionalism. Excellent written and oral communication. Ability to communicate effectively with providers. Completion of orientation and on the job training. Working conditions Works in a department setting as well as to and from outpatient clinics, emergency room, and inpatient. May be exposed to virus, disease, infections from patients. Physical requirements While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, reading electronic medical records and paper documents. This position requires prolonged periods of sitting with intermittent physical activity, including standing, walking, bending, kneeling, stooping, and crouching. Direct reports None
