Job Description

SUMMARY:
The Medical Records Clerk is responsible for maintaining accurate, complete, confidential, and timely patient medical records. This position ensures medical information is properly documented, filed, and released in accordance with clinic policies, HIPAA regulations, and applicable state and federal requirements. The Medical Records Clerk plays a vital role in supporting quality patient care, provider communication, insurance processing, and regulatory compliance. The quality and timeliness of medical records management directly impact patient care, provider decision-making, insurance reimbursement, regulatory compliance, and patient satisfaction. Therefore, accuracy, accountability, and effective communication are essential functions of this position.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Maintain, organize, and update patient medical records. Scan, stamp, and file documents accurately within the patient's EHR. Review records for completeness and identify missing or incomplete documentation. Ensure patient information is stored securely and confidentially. Verify that all documents are entered into the correct patient chart and appropriate section of the medical record. Monitor and process incoming requests and prioritize requests according to urgency and due dates and date received in a timely and efficient manner. Communicate proactively with leadership when there are workload challenges and request assistance BEFORE deadlines are missed. Communicate professionally with patients, providers, insurance representatives, and clinic staff. Return phone calls by the end of the next business day and document accordingly. Demonstrate accountability for responsibilities and assist with other duties as assigned. Protect patient confidentiality at all times. Understand that accurate documentation is essential for patient safety, continuity of care, insurance reimbursement, legal compliance. Errors may result in delays in treatment, claim denials, patient dissatisfaction, and compliance concerns. The above specified tasks may not be the only duties assigned. The medical records clerk will be required to carry out any other job-related functions requested by their supervisor subject to reasonable training and accommodations.
QUALIFICATION REQUIREMENTS

EXPERIENCE
  • High school diploma or equivalent.
  • 3-5 years of medical office experience preferred.
  • Experience working in an
EMR. PHYSICAL DEMANDS
Requires frequent mobility, prolonged standing, bending, stooping and stretching. May require lifting up to 50 pounds of boxes, paper, or charts. Must be able to reach above the head and extract up to 10 pounds of weight in charts. Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, the telephone, and other office equipment. Requires hearing within normal range for telephone use and listening to patients. Requires vision within normal range to read correspondence, charts, and computer screens.
OTHER SKILLS AND ABILITIES
  • Ability to perform with frequent interruptions and/or distractions.
  • Respond appropriately to changing situations.
  • Ability to establish and maintain working relationships with co-workers.
  • Must be able to do simple to moderate complexity math calculations.
  • Must be able to use initiative and independent judgement with established guidelines
  • Frequent mental alertness and attention to detail requires through work shift.

Possibility of exposure to biohazard materials and bodily fluids.
