Full Job Description General Summary:
A nonexempt position responsible for assembling patient information into patients' medical charts/records in accurate and complete manner.
Essential Job Responsibilities:
1. Collects medical record file folders from all medical assistants and processes the documents appropriately. 2. Scans, batches and files patient records accurately in the EHR. 3. Responds to requests for medical records information according to established policies and procedures. 4. Complies with federal HIPAA regulations and practice policies for the privacy and security of patient information; explains the privacy policy to patients as needed; maintains appropriate documentation of access to medical records. 5. Communicates as needed with physicians and other health care professionals. 6. Covers medical records front office desk providing excellent customer service to patients requesting information. 7. Enhances medical records and CNS reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments. 8. Performs other duties as assigned.
Education:
High school diploma.
Experience:
Minimum two years of medical records/health information technician experience, preferably in medical practice setting or any equivalent combination of experience, training and/or education approved by the Patient Access Manager.
Performance Requirements:
Knowledge:
1. Knowledge of medical terminology. 2. Knowledge and understanding of HIPAA Privacy and Security rules. 3. Knowledge of medical records processes. 4. Knowledge of health care field and medical office protocols/procedures.
Skills:
1. Skill in using computer and EHR software. 2. Skill in dealing diplomatically with the public under stressful conditions. 3. Skill in tact and diplomacy in interpersonal interactions.
Abilities:
1. Ability to work in a team based environment that places patient satisfaction as the primary focus for measuring success. 2. Ability to pay attention to detail. 3. Ability to effectively articulate information and respond to questions. 4. Ability to relate to and work well with a diverse community population. 5. Ability to work under pressure. 6. Ability to work cooperatively with other department staff. 7. Ability to project a pleasant and professional image. 8. Ability to identify problems and recommend solutions. 9. Ability to plan, prioritize, and complete delegated tasks in an appropriate timeframe. 10. Ability to demonstrate compassion and caring in dealing with others.
Equipment Operated:
Standard office equipment with emphasis on EHR system.
Work Environment:
Comfortable setting within the medical practice building. Well-lit, well-ventilated, adequate space.
Mental/Physical Requirements:
Combination of standing, sitting, bending, and reaching.
