  • Metropolitan, Health Information Management Department (HIMD), is accepting applications for the Health Record Technician I (HRT I), Permanent/Full Time position.
Under the direction of the Supervisor I or designee, the HRT I performs a variety of general office and clerical work in support of the Hospital's Vision, Mission, Goals, and Values. The HRT I is expected to consistently exercise a high degree of self-initiative and independence when performing assigned tasks. Responsibilities include but are not limited to:
CONCURRENT
Abstract, process, file, collate, and audit data from health records. Enter abstracted health record data into the Admissions/Discharge/Transfers (ADT) computer system and PLATO.
Chart Maintenance:
Label, repair and arrange. Code health records in accordance with DSM, ICD, and CPT coding guidelines: such as discharge summaries, psychiatric evaluations, ancillary services, treatment planning conferences, physician and psychologist billing services. Maintain log of completed work and perform other special audits and reviews related to health records functions. Deliver and retrieve health records between patient care areas and
HIMD. DISCHARGE
Abstract, process, file, collate, and audit data from health records. Enter abstracted health record data into the ADT computer system. Code health records in accordance with DSM, ICD, and CPT coding guidelines: such as discharge summaries, psychiatric evaluations, ancillary services, treatment planning conferences, physician and psychologist billing services. Locate, retrieve and file health records and documentation in all record storage areas. Maintain and purge files/documents in all archive areas. Process health records for admission, transfer or discharge. Maintain assigned filing areas in correct filing order for easy retrieval and access of health records. Assemble health records as a unit record; all prior admissions brought forward and filed under the new hospital admission number. Deliver and retrieve health records within the hospital and copy health records as assigned. Maintain a log of completed work and perform other audits and reviews related to health record functions. Training and Development (T&D): Individuals who are eligible for a T&D assignment may also be considered for this position. T&D assignments may be considered for current State employees. If you are a current State employee interested in a T&D assignment, you are not required to meet the Minimum Qualifications; however, you must indicate T&D as your eligibility when applying. Minimum Qualifications:
Either I:
One year of California state service clerical experience which includes training and supervised work experience in the procurement and processing of health data.
Or II:
Two years of clerical experience in the medical or health records section of a health facility. (Successful completion of an academic curriculum in medical records science in an accredited school may be substituted for the required general experience on a year-for-year basis.)
