Metrolina Nephrology Associates is the region's most recognized and experienced nephrology group. The practice has been on the forefront of the treatment and management of kidney disease for more than 40 years. A non-exempt position responsible for creating and maintaining organized and accurate electronic chart information that is made available when and where it is needed for patient care and continuity in a manner that is consistent with medical, ethical, legal and regulatory requirements. In addition, an important part of this position is to gather and distribute for signature, outside imaging reports and outside physician notes. Daily Health Information Management Duties
- Process incoming faxes (virtual or paper) and scan, file and task records to patient charts and providers.
- Obtain missing or incomplete patient information before forwarding or filing.
- Check to ensure all Release of Information (ROI) forms are signed as appropriate and legally binding per HIPAA guidelines.
- File records from scanned location batches.
- Work fax folders in EHR for each location daily, as assigned. This includes indexing into a patient's chart or into the appropriate file cabinets.
- Submits, via task to providers and others as instructed when indexing Outside MD Notes, Imaging Reports and Outside Labs for their review and signature.
- Performs document deficiency analysis with EHR report to ensure documentation is completed on arrived patient appointments; contact provider if no documentation completed.
- Reconciles daily hospital follow-up schedule to ensure consult notes and discharge summaries are available in MNA EHR
- Assists with daily departmental phone calls and may be assigned certain days to manage the calls
- Provide documents requested for specialist and transplant referrals, MNA office requests, dialysis units and patient requests as needed. Always follows HIPAA and MNA patient consent and confidentiality protocols.
- Sends documents to dialysis units as instructed, requested or as outlined in defined workflows
- Monitors and works failed faxes to correct incorrect fax information and identify transmissions errors and issues
- Weekly Mail (Interoffice and Postal) and Medical Records Mailbox (near Business Office)
- Performs the deceased patient workflow
- Compiles and uploads records for Risk Adjustment requests
- Chart merges for duplicate charts
- Chart analysis to ensure all documentation is completed in charts for arrived patients
- Acumen daily InBasket Messages Tasklist and Worklist Management
- Tasks are reviewed daily for requests for records and other items, including overdue orders. Tasks are addressed in a timely fashion to ensure continuity of patient care. Tasks are annotated each "touch".
- Overdue imaging orders are prioritized as the most important overdue orders worklist and worked daily. Appropriate teams are tasked when working these orders. These items are "worked" to obtain the reports and interpretation of MNA requested imaging studies.
- Overdue Referral Orders Awaiting Records are prioritized as the second most important overdue orders worklist.
Appropriate teams are tasked when working these orders. These items are "worked" to obtain the office notes, labs, etc. for MNA requested specialist consults.
Requirements:
Experience:
Minimum one-year experience working in a healthcare practice preferred. Possess basic knowledge of HIPAA and Release of Information rules and regulations.
Technology Skills:
Must possess skill in navigating systems used in the physician practice such as Practice Management Systems and Electronic Health Records (EHR), as well as different hospital EHRs. Proficient in Outlook. Competent in Learning Management Systems.
Attendance :
Regular, reliable, and punctual attendance is an essential function of this position. The ability to consistently adhere to scheduled work hours and fulfill job responsibilities in a timely manner is critical to the success of the role and the overall operations of the team.
