WHAT
YOU WILL DO
Prepares and maintains medical charts/records, forms and other related documents; reviews and compiles newly opened and ongoing cases for completeness and accuracy; obtains information required to complete opening process; maintains specific information from medical records/charts for quality improvement and billing purposes; scans patient information and documents as required. Safeguards confidentiality of the medical charts/records and complies with all state and federal laws pertaining to medical records; assures compliance with all laws concerning use, retrieval, storage and sharing of medical records. Copies and distributes status change, intake change and other requisite forms as required by agency standards; prepares and mails and/or faxes as required, correspondence for newly opened cases, recertification correspondence and telephone orders to attending physicians. Itemizes and files clinical charts according to status of patient; assembles clinical charts, maintenance records, and labels charts according to the status of the patient and file charts accordingly; requests patient history and physical information from hospital medical records as necessary; updates hospital admission and death files as situations occur throughout the day. Follows up on forms (by phone/mail/fax) that require signatures and are not in the file; maintains a tickler file of follow up activity; performs chart audits as directed. Assists with care conference preparation according to the support secretary practice and procedure manual. Answers telephone calls and handles requests for information; prepares and processes letters, memoranda, reports and other documents as assigned. Attends support staff meetings; receives and distributes mail; stocks and orders routine and special order items; performs general office work as assigned.
SUCCESS CAPABILITIES
High School Diploma or equivalent experience in an administrative setting, particularly in clinical records/medical records management, beneficial Knowledge of medical terminology Knowledge of the principles and practices of health information records management Ability to maintain accurate records and files Ability to review work for completeness and accuracy Ability to communicate effectively, orally and in writing Ability to establish and maintain effective working relationships with others Ability to set workload priorities and meet established deadlines Ability to maintain confidential information Maintenance of a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HRW's insurer Proficient in word processing, spreadsheet and database software, and in the use of general office equipment.
PHYSICAL REQUIREMENTS
This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions.
