- Full-time Health Information and Records Specialist Position Summary The Health Information and Records Specialist is responsible for the accurate organization, maintenance, security, and accessibility of patient health records in both electronic and paper formats.
In a Critical Access Hospital (CAH) and rural healthcare environment, this role supports multiple service lines (acute care, swing bed, long-term care, outpatient services, and ancillary departments) and requires adaptability to a wide scope of Health Information Management (HIM) functions. This position ensures the integrity, confidentiality, and availability of health information in compliance with federal and state regulations, including

Conditions of Participation for CAHs, and Wyoming state requirements. Essential Duties and Responsibilities Maintain complete, accurate, and timely patient health records in compliance with regulatory, legal, and organizational standards. Assemble, analyze, and reconcile medical records to ensure completeness and appropriate provider documentation. Monitor and manage medical record deficiencies and delinquency tracking to ensure timely provider completion. Conduct ongoing chart audits to ensure compliance with documentation requirements and standards. Perform data entry, scanning, indexing, and quality assurance functions within the electronic health record (EHR) system. Troubleshoot basic EHR issues and escalate system concerns as appropriate. Ensure accurate integration and continuity of records across care settings (acute care, clinic, long-term care, etc.) Support downtime procedures and recovery processes to ensure continuity of documentation. Process and fulfill requests for release of information (ROI) from patients, providers, attorneys, law enforcement, and government agencies in accordance with HIPAA and organizational policies. Ensure timely, accurate processing of legal documentation and correspondence. Maintain ROI tracking logs, audit trails, and related documentation. Safeguard the confidentiality and security of all protected health information (PHI) Support HIPAA compliance activities, including audits, monitoring, and breach prevention efforts. Participate in regulatory surveys, audits, and accreditation processes. Assist with compliance monitoring and development or revision of HIM-related policies and procedures. Complete and review birth certificate documentation for accuracy and completeness Ensure timely submission of birth records to Wyoming Vital Records in accordance with state requirements. Generate routine and ad hoc reports (daily, weekly, monthly, annual) related to HIM operations and metrics. Support reporting requirements for internal departments, CMS, and state or federal agencies. Assist with quality improvement initiatives and data abstraction activities as assigned. Assist providers and staff with accessing, retrieving, and interpreting medical records. Serve as a liaison between Health Information Management and clinical, administrative, and external stakeholders. Provide cross-coverage and clerical support, including scanning, faxing, filing, and front-end HIM functions. Provide training and guidance to staff on documentation standards, record management practices, and HIM processes. Maintain record retention schedules in compliance with federal and state regulations. Coordinate secure storage, archival, and destruction of medical records. Manage legacy paper records and hybrid record systems typical of a rural healthcare environment. Education and Experience
- High school diploma or equivalent
- Required. 1-3 years of experience in a healthcare setting OR administrative/records management role
