Job Description

Job Overview We are seeking a dedicated and detail-oriented Intake Specialist to join our team. The ideal candidate will play a crucial role in managing the initial contact with clients, ensuring that their needs are accurately assessed and addressed. This position requires strong organizational skills, excellent communication abilities, and a commitment to providing exceptional service.
Client Advocate:
Serve as the primary point of contact for returning citizens, ensuring they feel supported and understood. Advocate for clients' needs within the organization and with external service providers.
Needs Assessor:
Conduct thorough assessments to identify clients' immediate and long-term needs. Develop personalized reentry plans based on individual assessments.
Resource Coordinator:
Connect clients with internal and external resources to address their needs. Ensure seamless coordination of services, including housing, employment, education, healthcare, and mental health support.
Responsibilities Initial Client Interaction:
Greet and welcome returning citizens when they first walk in. Conduct comprehensive intake interviews to gather personal, criminal history, and needs assessment information. Explain the reentry program's services, policies, and processes.
Data Collection and Documentation:
Accurately record and maintain client information, including personal details, assessments, and service needs. Ensure all records are up-to-date, secure, and confidential.
Case Management:
Work closely with case managers and other stakeholders to ensure a comprehensive support network for clients.
Knowledge, Skills, and Abilities Knowledge:
Understanding of reentry challenges and the criminal justice system. Familiarity with community resources and services for housing, employment, education, healthcare, and mental health. Awareness of confidentiality and data protection regulations.
Skills:
Excellent verbal and written communication skills. Strong organizational and time management abilities. Proficiency in using case management software and databases. Critical thinking and problem-solving skills. Ability to use standard office equipment, including but not limiting to computers, telephone systems, copiers/scanner.
Computer literate:
Microsoft Office Suite, Outlook, and web-based services. Team player that can work independently.
Abilities:
Ability to build trust and rapport with clients. Capacity to manage multiple cases and prioritize tasks effectively. Capability to work collaboratively with a diverse team of professionals. Sensitivity to cultural differences and ability to work with diverse populations. Ability to advocate effectively for clients' needs. Must be able to navigate stairs and lift/move items up to 25lbs.
Qualifications Education:
High school diploma required. Bachelor's degree preferred in social work, criminal justice, psychology, or a related field is preferred.
Experience:
Previous experience working with formerly incarcerated individuals or in a reentry program is highly beneficial. Experience in case management or a related field is preferred.
Certifications:
Relevant certifications in social work or case management can be advantageous.
Other Requirements:
A valid driver's license and reliable transportation may be required. Ability to pass a background check. Compliance Comply with all Foundation of HOPE personnel policies and standards. Personally exemplify the values of the organization and its mission. Complete all required personnel paperwork in a timely and professional manner. Maintain flexibility and creativity to best meet the needs of our participants. Agree to and sign a Confidentiality Agreement regarding all aspects of client, staff, and agency information. Maintain professionalism in communications, personal interactions, and attire. Workday is 8:30-4:30pm, Monday to Fridays with the occasional need to modify schedule for events, trainings, or staff meetings.
