  • Shift differentials for shifts that start after 2PM. Schedules Full Time
  • AM Shift
  • SUN through THURS
  • 7:00AM to 3:30PM (40 Hours) Part Time
  • AM Shift
  • FRI and SAT
  • 7:00AM to 3:30PM (16 Hours) Part Time
  • PM Shift
  • THURS, FRI and SAT
  • 3:00PM to 11:30PM (24 Hours)

  • For positions scheduled 24 or more hours per week.
Position Summary Works at the direction of the Creekside Care Director nurse and Assisted Living Manager according to scheduled duties. Provides direct resident assistance with medication self-administration and activities of personal care as needed. Maintains medication and treatment areas. Primary Responsibilities Approaches all encounters with residents and co-workers in a friendly, service-oriented manner Maintains standards of personal appearance and grooming Works towards maintaining each resident's self-respect, personal dignity and personal safety Assists with medication set up and medication pass Observes, documents and reports changes in resident's condition Orders medications and tracks medications as necessary. Makes necessary calls to physician, pharmacy or family as part of follow up Documents medication received, released, and destroyed Verifies physician's orders. Monitors residents vital sign, weight, etc. related to medication management Prepares medication records for printing Performs monthly pharmacy audits Participates in resident care planning Maintains cleanliness of medication and treatment areas Monitors supplies Meets with RLA Nurse daily Performs other duties and tasks as assigned or required within the scope of the job Minimum Qualifications High School diploma or GED. Certified Nursing Assistant required. Must have current First Aid and CPR certification or be eligible. Completion of an approved RCFE Medication Assistance Training program with a passing grade. Obtain continuing education requirements to maintain all certificates. Must demonstrate excellent skills in reading writing, speaking and understanding the English language. Other languages a plus. Must be 21 years of age. Must be knowledgeable of pertinent regulations with monitoring and licensing of the facility. Knowledgeable of medical terminology.
