In this vital role, you will assist with medication administration, support daily living activities, and provide personalized care in assisted living, nursing homes, or home care settings. Your expertise will help promote independence, safety, and well-being for those you serve. Responsibilities Safely administer medications following prescribed instructions while adhering to HIPAA regulations and medical documentation standards. Monitor vital signs such as blood pressure, pulse, temperature, and respiration to assess patient health status. Assist residents or clients with daily activities including bathing, dressing, mobility, and nutrition support such as tube feeding. Maintain accurate medical records using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems like Epic or Cerner. Support patients with specialized needs including Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Perform catheter care, wound management, and other medical procedures under supervision or as trained. Observe and report changes in condition or behavior promptly to healthcare professionals to ensure continuous quality care. Experience Prior experience working in assisted living facilities, nursing homes, hospitals, or home & community care environments is highly desirable. Knowledge of medical terminology, physiology, anatomy, and nursing practices related to medication administration and patient care. Familiarity with EMR/EHR systems such as Epic or Cerner for documentation purposes. Experience caring for individuals with complex needs including Alzheimer's disease, dementia, or other cognitive impairments. Demonstrated ability to work compassionately with people of all ages who have disabilities or chronic health conditions. Certification in medication administration (e.g., CNA license or equivalent) is preferred but not mandatory; training opportunities are available for qualified candidates. Strong understanding of HIPAA compliance and medical records management to ensure confidentiality and data security.
