Your expertise will ensure that individuals with diverse needs—ranging from memory care to hospice support—receive the highest standard of medication management and patient care. This position offers an opportunity to make a meaningful difference in the lives of those you serve while working within a dynamic, supportive environment committed to excellence. Duties Safely administer prescribed medications to residents following all applicable protocols and physician orders, ensuring compliance with HIPAA and medical documentation standards. Monitor residents' vital signs, physiological responses, and overall health status before and after medication administration. Maintain accurate medical records using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems such as Epic or Cerner, documenting medication details and patient responses meticulously. Assist with specialized care procedures including tube feeding, catheter care, and managing medications for individuals with dementia, Alzheimer's, or other memory impairments. Collaborate with nursing staff and healthcare providers to update care plans, report changes in condition, and ensure seamless communication across the care team. Support residents with activities of daily living in assisted living or home care environments, emphasizing dignity and personalized attention. Follow infection control protocols and ensure medication storage complies with safety standards to prevent contamination or errors. Proven experience working with individuals with developmental disabilities, Alzheimer's disease, dementia, or in hospice and memory care settings. Strong knowledge of physiology, anatomy, medical terminology, and medication administration procedures. Familiarity with EMR/EHR systems such as Epic or Cerner; experience with medical documentation and recordkeeping is highly preferred. Ability to accurately measure vital signs including blood pressure, pulse, respiration rate, and temperature. Excellent caregiving skills combined with compassion for seniors and individuals with disabilities; patience and strong communication skills are essential.
