  • Summary In this role, you'll collect and document accurate medication histories for patients being admitted to the hospital.
You'll work under pharmacist supervision and collaborate closely with nurses, providers, and other care team members.
Key responsibilities:
Interview patients, caregivers, pharmacies, and provider offices to gather complete medication information-Document prescription medications, over-the-counter products, allergies, and compliance details accurately-Enter medication histories into the electronic medical record (EMR) to support reconciliation-Identify and help resolve discrepancies in medication information alongside pharmacists and nurses Maintain patient confidentiality and adhere to organizational and regulatory standards-Interview patients, caregivers, pharmacies, and provider offices to gather complete medication information Document prescription medications, over-the-counter products, allergies, and compliance details accurately Enter medication histories into the electronic medical record (EMR) to support reconciliation Identify and help resolve discrepancies in medication information alongside pharmacists and nurses Maintain patient confidentiality and adhere to organizational and regulatory standards
Requirements Education:
High School diploma or equivalent required.
Experience:
One year of hospital pharmacy experience preferred. One year of Pharmacy Technician experience preferred.
Certification:
CPht (Certified Pharmacy Technician) certification with the PTCB (Pharmacy Technician Certification Board) required (must be obtained within first 180 days of hire). ExCPT certification may be accepted on a case-by-case basis.
License:
Registered Pharmacy Technician with Florida Department of Health required.
Other:
Data entry and knowledge of pharmaceutical terminology. Ability to cope with confrontation.
