Min $21.68 Mid $27.10 Position Summary The Medication History Pharmacy Technician works within the scope of pharmacy laws and standards of practice to complete and document the best possible medication history. The technician performs this function by interviewing patients and/or family members, utilizing the electronic health record and outpatient pharmacy information, as well as contacting outside pharmacies or healthcare facilities. Follows department standard operating procedures to perform job functions and documentation requirements. Communicates medication history information with pharmacy staff and the multidisciplinary team as appropriate.Position Responsibilities Standard Work Duties:
Medication History Pharmacy Technician Under the direction of pharmacy leadership, actively participates in pharmacy team development and accomplishing departmental goals, objectives, and priorities. Performs all duties as assigned, which may include traditional pharmacy technician tasks. Efficiently identify patients in the ED and those admitted to the floor who need a medication history completed. Evaluate patient information relevant to obtaining medication history (patient communication ability, outpatient pharmacy information, EFORSCE, etc) in an effort to obtain the best possible medication history. Contact outpatient pharmacies or other healthcare entities when necessary. Conduct a thorough medication history interview with patient and/or family member(s). Refer medication questions (such as those regarding side effects, interactions, etc), discrepancies, and potential errors/adverse events to pharmacist; appropriately triage questions or inquiries from the patient or families outside the scope of taking the medication history to the appropriate caregivers or staff. Document an accurate medication history for each patient, including, but not limited to: proper identification of dose/route/frequency/dosage form, as well as prescription and any non-prescription/herbal medications. Record patient preferred pharmacy, allergy assessment, special medication information, and all other documentation requirements as outlined by SOP/standard work. Organizes job functions and work assignments to be able to effectively complete the minimum number of medication histories per shift, while maintaining accuracy and quality. Identifies and participates in ongoing quality assurance to ensure efficiency and accuracy for all aspects of medication history taking. Trains pharmacy technicians and interns if deemed appropriate by the supervisor/manager. Remains current with all LRH and departmental mandatory education. Minimum of two years of pharmacy technician experience.Experience Preferred:
Minimum of four years of pharmacy technician experience. Experience with conducting medication histories.Certifications Essential:
Certification by the national Pharmacy Technician Certification Board (PTCB) is required upon hire or within 6 months of hire.Licenses Essential:
