33788 Medication Reconciliation Pharmacy Technician Job Summary The Medication Reconciliation Pharmacy Technician is responsible for working with newly admitted patients in the ED to obtain and document an accurate home medication list to help avoid medication duplications, omissions, and errors. Incumbent obtains and enters information used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. This position is also responsible for handling the drug distribution system and providing technical support in all aspects of the delivery of safe, effective, patient-focused, and cost-efficient drug therapy, and also assisting the pharmacist in the daily functions of the pharmacy department. Qualifications High School Diploma or GED Required Vocational/Technical Degree in Pharmacy technology or related field Preferred Work Experience 1 or more years retail or hospital-based pharmacy experience Required Licenses and Certifications LPT - Licensed Pharmacy Tech Technician Registration with the Georgia State Board of Pharmacy Required PHAR-C - Certified Pharmacy Tech Required Essential Functions Responsible for assisting pharmacists in receiving, processing, and dispensing of all physicians' orders for medications, IV solutions, and outpatient prescriptions. Transports drugs and supplies to and from the pharmacy area to ensure delivery of quality care according to established protocols and transport times. Completed training in aseptic technique and performs duties and responsibilities of an IV technician. Responsible for answering phones, window, and doors, also loading and unloading the pneumatic tube system. Assist in pulling out dates from main pharmacy and automated dispensing cabinets. Assist in stock replenishment and maintaining inventory. Works with pharmacists in performing monthly nursing station audits. Obtains and clarifies patient's home medication list by utilizing Meditech and various drug information resources. Contacts community pharmacies, physician offices, or other sources as appropriate for prescription clarification. Interviews patient/family and reviews prescription bottles brought to the Emergency Department by the patient to ensure proper identification of doses and drugs including prescription medications, over the counter drugs and herbals/supplements. Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate. Serves as a liaison between pharmacy and nursing to foster medication teaching the patient in a required time frame, complies with Pharmacy and hospital policies and procedures. Responsible for resolving and ordering missing medications through central pharmacy. Help facilitate transfer of medications for patients moving off the unit. Checks medication storage areas for discontinued and expired medications and facilitates return to pharmacy. Credits medications and returns to pharmacy for return to stock. Restocks Automated Dispensing Cabinet, performs monthly inspection for accrediting body readiness. Volunteers and assists as needed during staffing emergencies and other occurrences that may create unusual staffing circumstances. Communicates with physicians and nurses professionally and courteously on medication related issues. Takes ownership of problems by reporting issues to appropriate hospital staff to achieve desired outcome. Involves pharmacy supervisory personnel when necessary to reach goal. Demonstrates technical skill in using equipment appropriate for role. Understands equipment operation. Maintains equipment in working order. Troubleshoots equipment problems. Uses technology to increase productivity. Adapts to new technology.
