RESPONSIBILITIES I. JOB SUMMARY
Conducts interviews with patient and/or caregivers, contacts outpatient providers/pharmacies, and utilizes other resources to obtain the most complete and accurate list of all medications that the patient is taking (including name, dosage, frequency route, and last dose) prior to admissions to the hospital. Enters medication list into the inpatient electronic medical record. Escalates any discrepancies that need further clarification to the covering clinical pharmacist. Performs duties of a Pharmacy Technician as needed.

III. TYPICAL WORKING CONDITIONS
Not substantially subjected to adverse environmental conditions. Some exposure to cold temperature from freezer. Exposure to noise/vibration from air conditioner, and office/pharmacy equipment. Exposure to body fluids, chemicals, contact with patients with communicable diseases. May be required to work with hazardous drugs, depending upon area of assignment. May be required to work evening or night shift. Work environment is fast paced, congested and may be stressful.
IV. MINIMUM QUALIFICATIONS
A.
EDUCATION
High School diploma or equivalent.
B. CERTIFICATION AND LICENSURE
Current Hawaii State certificate of registration as a Pharmacy Technician. Completion of pharmacy technician program preferred. Certification by the Pharmacy Technician Board (PTCB) preferred.
C. EXPERIENCE
Six (6) months work experience as a pharmacy technician; or three (3) months work experience as a pharmacy technician with completion of a pharmacy technician program.
Experience to demonstrate:
o Knowledge of drug names (both trade and generic), uses, dosages, and routes of administration. o Knowledge of medical terminology and abbreviations. o Ability to read, write, speak and understand English. o Ability to effectively communicate with both patients and other healthcare professionals. o Ability to type, operate calculator and computer. o Ability to perform pharmacy calculations involving ratio/proportion, IV flow rate calculations, and dosage calculations including daily doses, and days of supply calculations.
