What We're Looking For:

Strong attention to detail with a commitment to accuracy Ability to work independently and prioritize effectively in a fast-paced environment Customer-service oriented with excellent interpersonal and communication skills Working knowledge of brand and generic drug names, uses, doses, and formulations High school diploma or equivalent required Two (2) years of technician experience in a retail or hospital setting/health system required Three (3) years of pharmacy tech experience in a hospital environment preferred National PTCB (CPhT) certification required upon hire State of Louisiana Pharmacy Technician Licensure required upon hire Pharmacy Technician Career Ladder (Assessment Certificates) required within 60 days of hire CPR/BLS certification required within 60 days of hire

What You Will Do:

The Medication History Technician ensures safe and thorough patient care by collecting and documenting comprehensive medication histories for patients being admitted to the hospital. Obtain complete and current medication histories from patients through direct interviews, caregiver conversations, and pharmacy or hospital record reviews Conduct telephone interviews for upcoming surgery patients to gather medication information Communicate with external pharmacies, physicians, and insurance companies to gather drug names, dosages, and administration details Enter complete medication history into the hospital's electronic health record for physician review and reconciliation Coordinate with nurses, physicians, and case managers to prioritize patients for medication interviews Identify and resolve discrepancies in medication histories, escalating as needed to pharmacists or physicians Perform quality audits of medication histories to ensure accuracy and completeness Assist in the Emergency Department with medication distribution and monthly unit inspections Maintain productivity standards in a dynamic and patient-centered environment
