Job Description

Overview Join Evera at Hoyt Lakes healthcare team as a Trained Medication Aide, where your expertise will make a meaningful difference in the lives of our residents. In this vital role, you will assist with medication administration, support patient care, and promote a safe, comfortable environment for individuals across various care settings. Your skills in nursing assistance, medication management, and compassionate caregiving will help enhance quality of life for seniors and individuals with disabilities. Responsibilities Safely administer prescribed medications to residents following all HIPAA regulations and medical documentation standards. Monitor and record vital signs, medication effects, and patient responses accurately using electronic health record (EHR) system Eldermark. Assist with activities of daily living, including hygiene, mobility support, and feeding, especially in assisted living, memory care, or hospice environments. Support residents with specialized needs such as tube feeding, catheter care, or dementia-related behaviors within a compassionate framework. Maintain detailed medical records and document all patient interactions clearly and thoroughly in compliance with healthcare protocols. Collaborate with nursing staff and physicians to ensure optimal patient care plans are followed and updated regularly. Observe for signs of deterioration or changes in condition related to physiology or anatomy knowledge, reporting findings promptly to the healthcare team. Experience Prior experience working in assisted living facilities, nursing homes, hospitals, or home care environments is highly preferred. Demonstrated proficiency with EMR/EHR system Eldermark is advantageous. Knowledge of medical terminology, anatomy, physiology, and medication administration procedures is essential. Experience caring for individuals with developmental disabilities, Alzheimer's disease, dementia, or memory care needs is a plus. Background in hospice or acute care settings provides valuable insight into complex patient needs. Strong caregiving skills combined with an understanding of HIPAA regulations and medical documentation standards are required. Previous roles involving medical office work or home & community care experience will support success in this position. Pay:
$20.80 - $24.06 per hour
