Job Description
Job Summary The Pharmacy Manager, or designee, the Pharmacy Medication History Tech is responsible for gathering, documenting, and verifying comprehensive medication histories for patients upon admission to healthcare facilities. The Pharmacy Medication History Tech will use a multitude of resources to gather patient medication information obtaining medication administration records to confirm accurate outpatient pharmaceutical regimens.
Core Job Responsibilities Performs services and functions within assigned area(s) where complex pharmaceutical processes require the utilization of advanced skills and knowledge beyond those routinely required across pharmacy technician roles
Interviews patients, family members, and caregivers to obtain a complete and accurate history of prescribed medications, over-the-counter drugs, supplements, and herbal remedies.
Cross-references patient-reported information with pharmacy records, electronic health records (EHR), and other available data sources.
Identifies discrepancies in medication histories and communicates them to the healthcare team for follow up and resolution.
Accurately documents medication histories in the patient's EHR, ensuring all relevant details, such as dosage, frequency, and route of administration, are included.
Problem solves complex problems and ensures that issues are escalated and closed as needed
Acts as a liaison between patients, healthcare providers, and pharmacies to clarify and verify medication details.
Works closely with physicians, nurses, pharmacists, and other healthcare providers to ensure the accuracy of medication histories.
Participates in initiatives to improve the medication reconciliation process and reduce medication errors
Contribute to the development and implementation of best practices and protocols for medication history gathering.
As a member of a High Reliability Organization (HRO), strives to make patient safety a top priority and uses excellent customer service skills in all interactions.
Perform related duties as assigned. Education/Experience Requirements
REQUIRED
High school diploma or equivalent. 1 year of previous hospital or retail pharmacy technician experience. In depth knowledge of generic and brand medication names and dosing for common illnesses. Knowledge of common disease processes. Excellent interpersonal and communication skills to interact with colleagues, providers, visitors and patients. Accurate and good attention to detail.PREFERRED
Fluency in a second language. Licensure/Certification RequirementsREQUIRED
PTCB certified/NYS registered pharmacy technician.
