Medication History Specialist | Part Time Technicians / Patient Care Support Pharmacy
DESCRIPTION

Shift Information:
This position is part time working 3 days per week.

Communicates with the patient, patient family or caregiver, retail pharmacies, physician offices and other facilities to provide the most current list of medications, dosages, dosage forms and frequency of administration. Supports and participates in hospital strategic and quality initiatives.
Required Education:
High School diploma or equivalent
Required Skills:
Computer order entry
Required Licensure/Registration/Certification:
Texas Pharmacy Technician Registration, Licensed Vocational Nurse, Registered Nurse or Medical Assistant Required Experience:
Prior experience with home medication history collection and knowledge of principles related to medications and direction for use.
