In this vital role, you will assist with medication administration, support daily living activities, and provide exceptional care in various settings such as assisted living facilities, nursing homes, hospitals, or home care environments. Your expertise will help promote health, safety, and well-being for those with diverse needs, including seniors, individuals with disabilities, and those experiencing memory loss or chronic conditions. This paid position offers an engaging opportunity to grow professionally while delivering heartfelt support to our clients. Duties Safely administer medications following prescribed instructions and document all medication activities accurately using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems such as Epic or Cerner. Monitor vital signs including blood pressure, pulse, respiration, and temperature to assess patient health status regularly. Assist with activities of daily living such as bathing, dressing, grooming, mobility support, and feeding—especially for individuals with Alzheimer's disease or other forms of dementia. Provide specialized care for memory care residents and those receiving hospice or palliative care by implementing tailored routines that promote comfort and dignity. Maintain detailed medical documentation in compliance with HIPAA regulations and ensure all records are current and accurate for medical records review. Support patients during tube feeding procedures or catheter care with precision and adherence to safety protocols. Collaborate with nursing staff and healthcare professionals to update care plans and communicate patient needs effectively. Skills Proven experience in assisted living facilities, nursing homes, hospitals, or home care environments. Strong knowledge of medical terminology, physiology, anatomy, and nursing practices related to medication administration and patient care. Familiarity with EMR/EHR systems such as Epic or Cerner for accurate documentation of medical records and medication logs. Ability to work effectively with individuals who have developmental disabilities or are experiencing cognitive decline due to Alzheimer's or dementia. Excellent caregiving skills emphasizing compassion, patience, and clear communication. Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information. Experience in hospice or memory care settings is highly desirable to provide specialized emotional support.
