Job Description

Job Overview We are seeking a dedicated and compassionate Certified Medication Technician (CMT) to join our skilled nursing facility for the PM Shift (10pm to 6am) . In this vital role, you will be responsible for administering medications and providing essential patient care to individuals in skilled nursing facility environments. Your expertise will ensure safe medication management, support residents' well-being, and promote a high standard of quality care. This position offers an exciting opportunity to make a meaningful difference in the lives of those you serve while working within a dynamic and supportive team. Responsibilities Administer prescribed medications accurately following physician orders and established protocols Monitor and record vital signs, medication effects, and patient responses in electronic health record (EHR) systems such as Epic or Cerner Assist with activities of daily living, including mobility support, hygiene, and nutrition assistance Maintain comprehensive medical documentation in compliance with HIPAA regulations and facility policies Observe and report any changes in patient condition, including symptoms related to Alzheimer's or dementia care Collaborate with nursing staff and other healthcare professionals to deliver holistic patient-centered care in assisted living, hospice, or acute care settings Skills Proven experience working with individuals with developmental disabilities, memory care needs, or senior populations Strong knowledge of physiology, anatomy, medical terminology, and medication administration protocols Familiarity with EMR/EHR systems such as PointClick Care, Epic or Cerner for accurate documentation of patient records and medical history Ability to perform vital signs assessment, catheter care, tube feeding assistance, and other clinical tasks with precision Excellent communication skills to effectively interact with patients, families, and healthcare teams Understanding of HIPAA compliance to ensure confidentiality of medical records and sensitive information Experience in SNU/SNF environments Join us in delivering compassionate care that enhances quality of life!
