Data collection Psychotherapy Full Job Description Overview We are seeking a dedicated Per Diem Field Medical Social Worker (MSW) specializing in Home Health services to join our healthcare team. This role involves providing comprehensive social work support directly within patients' homes, ensuring personalized care that addresses both medical and psychosocial needs. The ideal candidate will have a strong background in clinical and behavioral health, with experience working with diverse populations including seniors, individuals with disabilities, and those requiring specialized care such as dementia or autism. This position offers flexible scheduling and the opportunity to make a meaningful impact on patient outcomes through compassionate, patient-centered support. Responsibilities Conduct thorough patient assessments in home settings to evaluate social, emotional, and environmental factors affecting health. Develop individualized care plans that incorporate behavioral health interventions, crisis management, and resource coordination. Provide clinical counseling including mental health support, motivational interviewing, and psychotherapy tailored to patient needs. Assist patients and families with navigating healthcare systems, including intake procedures and medical documentation. Collaborate with interdisciplinary teams to coordinate services such as hospice care, senior care, or post-acute rehabilitation. Collect and document data accurately using Electronic Medical Record (EMR) systems while ensuring compliance with HIPAA regulations. Offer crisis intervention and behavioral therapy for patients experiencing mental health crises or behavioral challenges. Support patients with developmental disabilities, Alzheimer's disease, PTSD, or other complex conditions through specialized counseling techniques. Participate in clinical research projects related to home health or behavioral health outcomes when applicable. Maintain ongoing communication with healthcare providers regarding patient progress and case management updates. Skills Proven experience in child & family counseling, behavioral therapy, or clinical social work within home health or community settings. Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and familiarity with medical records management. Strong understanding of HIPAA compliance and confidentiality standards in healthcare documentation. Skilled in crisis management, diagnostic evaluation, and working with individuals with disabilities or developmental challenges. Competence in utilizing EMR systems for data collection and documentation purposes. Excellent interpersonal skills to engage effectively with patients across diverse backgrounds. Ability to perform comprehensive patient assessments and develop tailored intervention strategies. Experience working with seniors in assisted living environments or nursing homes is preferred. Familiarity with outpatient services, inpatient care settings, and post-acute care processes. Capable of managing multiple cases efficiently while maintaining attention to detail.
