The Licensed Behavioral Health Specialist supports El Sol Neighborhood Educational Center's mission by providing behavioral health services, care coordination, crisis intervention, and health education to individuals and families served through Enhanced Care Management (ECM) and other community-based programs. The Licensed Behavioral Health Specialist is responsible for supporting Members with behavioral health conditions by utilizing brief therapeutic interventions, motivational interviewing, crisis response coordination, and community resource linkage to improve overall health outcomes and wellness. This role requires strong clinical judgment, organizational skills, cultural responsiveness, and the ability to engage effectively with diverse populations in community and clinical settings.
Minimum requirements:
1-2+ years of related/equivalent experience with a Master's degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related behavioral health field from an accredited institution. Bilingual (Spanish/ English; speak and write) preferred. Must clear FBI/DOJ background check.
Qualifications:
Minimum of 1+ years of experience providing behavioral health services, care coordination, case management, or community-based support services preferred. Knowledge of evidence-based behavioral health practices, including Motivational Interviewing, brief interventions, crisis intervention, and trauma-informed care principles. Strong organizational, documentation, and case management skills with attention to detail and accuracy in data entry and record management. Ability to multitask, prioritize responsibilities, adapt to changing priorities, and work effectively in a fast-paced environment. Ability to collaborate effectively within a multidisciplinary care team and build positive working relationships with internal and external partners. Knowledge of local social services, behavioral health systems, and community resources available to support Members' health and social needs. Experience supporting health education and behavioral health programs. Ability to work effectively within multicultural and underserved communities; familiarity with and connection to the community served preferred.
Responsibilities:
Manage a primary caseload of ECM Members with behavioral health needs, focusing on Tier 1 and Tier 2 Members. Maintain a minimum caseload of 50 members. Conduct behavioral health screenings, assessments, brief therapeutic interventions, and crisis intervention services using evidence-based practices, including Motivational Interviewing and trauma-informed approaches. Provide "warm hand-offs," referrals, and care coordination with crisis response agencies, behavioral health providers, hospitals, PCPs, specialists, and community resources to ensure timely access to medical, behavioral health, and supportive services. Support care transitions by coordinating with psychiatric hospitals, the IEHP Transitions of Care Team, and other providers to ensure continuity of care following admissions and discharges. Educate Members and families on chronic medical and behavioral health conditions, treatment options, wellness strategies, and available community resources to improve health literacy and self-management skills. Partner with Members to establish individualized health and wellness goals and incorporate them into Shared Care Plans to support coordinated, person-centered care. Monitor and track medical and behavioral health outcomes through patient registries, electronic health records, and other documentation systems to support treatment effectiveness and quality improvement efforts. Collaborate with interdisciplinary care team members through regular case reviews, care conferences, and consultations to support integrated care planning and service delivery. Assist the RN Care Manager and interdisciplinary team by reviewing Comprehensive Health Assessments (CHAs) and providing behavioral health insight and recommendations as needed. Maintain accurate, timely, and compliant documentation in accordance with agency policies, HIPAA requirements, and program standards. Provide guidance and support to Members, families, staff, and partners as appropriate. Performs other duties as assigned.
