Job Title - MA II/Certified Peer support Specialist Location - New Castle, DE / Dover, DE/ Sussex, DE Contract Role Onsite Role Job Summary
- An individual who has lived experience with mental illness, addiction, co-occurring conditions, and/or recovery.
- Support others going through similar life challenges they once experienced.
- Assist individuals in developing the skills and resources they need to live an independent and productive life in the community.
- Helps consumers as they transition from one level of care to another. Key Responsibilities
- Interview's patients, clients, family members and/or persons deemed necessary to obtain the most complete case history.
- Assists in identifying, documenting, and advocating for the needs, goals, and problematic barriers that clients are facing.
- Maintains a caseload, providing support to clients and families.
- Prepares narrative report writing to document contacts. Required
- Assist in developing natural supports and community living by learning multiple roles (friend, spouse, employee, student, etc.) so a greater sense of identity is established.
- Must have direct personal experience living with mental illness.
- Must have a High School Diploma or GED.
- Partners with clients to ensure collaboration with current supporters identified i.e. family, friends, clinical home, current treatment services provider, housing supports, etc. to obtain collateral information and ongoing participation in identified recovery journey.
- May prepare application forms for benefits or services.
- Conducts routine follow-up calls to maintain contact with clients.
