Psychiatric Mental Health NP or Physician Assistant (PA) - Post-Acute Care/Facility Rounding

Florida Psychiatry & Wellness is seeking a Psychiatric Nurse Practitioner and/or a Physician Assistant (PA) to provide psychiatric evaluations and medication management while rounding at multiple post-acute care facilities. This role offers full autonomy with strong support.

Location(s): Orange Park, FL, Green Cove Springs, FL, or Jacksonville, FL Flexible Hours Florida Psychiatry & Wellness is seeking a compassionate, motivated, and clinically strong Psychiatric Nurse Practitioner (PMHNP preferred; FNP with psychiatry experience considered) or a Physician Assistant (PA) with psychiatric experience to join our growing practice. Position Overview:

Provide psychiatric evaluations, medication management, and follow-up care while rounding at multiple post-acute care facilities. This is an excellent opportunity for a provider who values independence, clinical excellence, and meaningful patient care in transitional settings.

Responsibilities:

Conduct comprehensive psychiatric evaluations and ongoing assessments Diagnose and treat mental health conditions Provide medication management and individualized treatment planning Round at several post-acute care facilities and collaborate with facility teams Maintain accurate and timely documentation Educate patients and families on diagnoses, medications, and treatment plans Work collaboratively with interdisciplinary teams to ensure continuity of care

Qualifications:

Active and unrestricted Florida Nurse Practitioner or Physician Assistant (PA) license Board Certification (PMHNP preferred) Active DEA license (or ability to obtain) Strong clinical judgment, professionalism, and communication skills Experience in Psychiatry strongly preferred
