  • Part-time, Full-time Job Overview Join our dedicated team as a Community Based Rehabilitative Specialist, where you will play a vital role in empowering individuals with mental disabilities, autism, and other behavioral health challenges to achieve greater independence and community integration.
This rewarding position involves providing personalized support, implementing behavior management strategies, and fostering meaningful connections within the community. Your efforts will directly enhance the quality of life for those you serve, promoting growth, safety, and well-being in their everyday environments. Duties Deliver direct support services to individuals with mental and/or physical disabilities, including assistance with activities of daily living (ADLs), personal care, and household tasks. Develop and implement individualized care plans that incorporate behavioral health techniques and special education strategies. Assist with communication needs using tools such as American Sign Language or other augmentative communication methods. Support clients in social skills development, community outings, and recreational activities to promote social inclusion. Collect and maintain accurate data on client progress, behavior management interventions, and service delivery for reporting purposes. Administer medications as prescribed following proper protocols and document all medication-related activities diligently. Drive clients safely to appointments, community events, or outings while maintaining a supportive and engaging environment. Collaborate with families, caregivers, and multidisciplinary teams to ensure comprehensive care and consistent support strategies. Qualifications Experience working with individuals with mental health disabilities, autism spectrum disorder, or behavioral health needs. Knowledge of skill development and behavior management techniques. Proficiency in American Sign Language is a plus; strong communication skills are essential. Background in home and community care settings with experience assisting with ADLs and caregiving tasks. Ability to collect data accurately and maintain detailed records of services provided. Valid driver's license with a clean driving record for safe transportation duties. Strong interpersonal skills to foster trust and positive relationships with clients and their families.
