Reporting to the Manager Psych & Behavioral Health the Mental Health Worker is an individual contributor that provides an array of supportive services designed to meet the needs of clients who are chronically mentally ill and/or developmentally disabled for the purpose of increasing social skills and rehabilitation back into the community. This work is conducted under the direct supervision of a professional staff member.Key Job Activities:
Provides supportive group, individual, and crisis interventions Provide Interpersonal skills when working with patients, including having compassion and remaining sensitive to their emotional needs when they are in extreme distress or pain Monitoring patients to ensure treatment plans are being followed and that the patient is taking all necessary medication(s) Monitors the patient's medication compliance and facilitates any scheduled medical appointments Ensuring patient safety and wellbeing while remaining dedicated to identifying ways to improve the patient's health and wellbeing Charting patient behavior and documenting any changes in behavior while ensuring that all progress notes meet documentation time guidelines on a 95% basis Willing and able to attend Supervision Sessions Participates in staff meetings, mandatory and designated in-service training sessions Ability to educate patients in group settings and providing various services in the community Ability to engage a patient therapeutically to help them learn healthy aspects of interpersonal social skills, hygiene and grooming and teamwork in social settings Perform all other duties as assignedEducation and Work Experience:
High school diploma or GED required; Associates degree and above highly preferred 5 Years of experience in leading or co-leading group and or mental health workSimilar jobs in Chicago, IL
