High Needs-Program Coordinator (PC) & Program Coordinator Assistant Divine House, Inc. is seeking kind, compassionate and responsible individuals to work as a Program Coordinator (PC) & Program Coordinator Assistant to supervise all aspects of the daily operation of a licensed group home site in Willmar, MN. The qualified candidates will have the ability to effectively supervise direct care staff to ensure the health, safety, and protection of rights of each person served in the program. They will also be responsible for overseeing the implementation of the person's served Coordinated Service and Support Plan (plan of care). They will ensure that they and their staff are providing the necessary support to the individual to ensure that their needs, interests, and preferences are respected and provided for in a safe and healthy environment that maximizes personal success. They must have the ability to communicate effectively, orally and in writing, and be able to create documents that are legible and accurate. Must have past experience with positive reinforcement, behavioral interventions, as well as providing persons served with personal cares and intermediate medical needs.
