Intensive In-Home Qualified Professional (Enhanced Services/Behavioral Health) Sunrise Clinical Associates, PLLC - 3.0 Raleigh-Durham, NC Job Details Part-time $22 - $25 an hour 8 hours ago Benefits 401(k) Flexible schedule Qualifications Spanish Sociology Driver's License Working with children Bachelor's degree Human Services Full Job Description The Intensive In-Home Qualified Professional (QP) provides community-based behavioral health services to children, adolescents, and families experiencing significant emotional, behavioral, or mental health challenges. This role supports treatment implementation, skill-building, crisis response, and care coordination in alignment with individualized Person-Centered Plans (PCPs) and service definitions under North Carolina Medicaid guidelines. Key Responsibilities Provide intensive in-home interventions to children and families in home, school, and community settings. Implement service strategies outlined in the Person-Centered Plan under the supervision of the Team Lead. Support families in developing coping skills, communication strategies, behavior management, and crisis prevention techniques. Coordinate care with schools, medical providers, DSS, juvenile justice, and other community stakeholders. Conduct strengths-based assessments and contribute to service planning. Maintain frequent contact with families to monitor progress and adjust interventions as needed. Assist with crisis intervention and stabilization to reduce risk of hospitalization or out-of-home placement. Complete timely and accurate documentation, including progress notes, incident reports, service plans, and authorizations. Participate in Child and Family Team meetings and interdisciplinary staffing. Ensure compliance with Medicaid, state regulations, and agency standards. Minimum Qualifications Bachelor's degree in Human Services field (Psychology, Social Work, Counseling, Sociology, or related field). Meets North Carolina Qualified Professional (QP) status requirements. Minimum of 2 years of experience working with children/adolescents with mental health, behavioral, or substance use needs. Valid driver's license, reliable transportation, and ability to travel regularly in the community. Strong knowledge of family systems, crisis intervention, and community resources. Excellent communication, organizational, and documentation skills. Preferred Qualifications Experience in Intensive In-Home, CST, or other Medicaid-enhanced services. Bilingual (English/Spanish) preferred. Knowledge of trauma-informed care and evidence-based interventions. Work Environment Community-based role with travel to client homes, schools, and community locations. Flexible schedule including evenings and occasional weekends based on family needs. Fast-paced environment requiring responsiveness to crisis situations.
