Job Description:
Responsibilities Provides IMR and crisis intervention services to assigned consumers on caseload Assists consumers with financial management and continuous linkage to community resources and benefits Coordinates medical appointments, assists with activities of daily living skills, and meeting program guidelines Implements prescribed treatment plan and assists in the development of IRPs Must be able to work independently and travel to consumers' residencies as required Qualifications BSW in social work, BA/BS in psychology BA/BS in a related field with two years of experience in mental health Valid New Jersey driver's license
