Licensed Substance Abuse Associate (LSAA) or Licensed Mental Health Counselor (LMHC) Addiction Recovery Support TriUnity Healthcare | Full-Time | Albuquerque, NM About TriUnity Healthcare TriUnity Healthcare is a pioneering addiction medicine program delivering Intensive Outpatient Program (IOP)-level care inside skilled nursing facilities (SNFs). We fill a critical gap by providing comprehensive, evidence-based treatment during the high-risk post-acute phase after hospital discharge — when patients need support the most. Our integrated model combines medication management, behavioral health, and recovery services in a collaborative, team-based environment focused on dignity, stability, and long-term recovery. Job Summary We are seeking a compassionate and dedicated Licensed Substance Abuse Associate (LSAA) or Licensed Mental Health Counselor (LMHC) to join our innovative addiction medicine-focused Skilled Nursing Facility (SNF). Our facility specializes in supporting patients during the critical first month of recovery following discharge from acute hospital care. In this role, your primary responsibilities will be facilitating Intensive Outpatient Program (IOP) groups and providing individual counseling sessions. You will lead daily therapeutic groups using evidence-based curricula, deliver one-on-one counseling to support patients' recovery journeys, and collaborate with our multidisciplinary team to ensure comprehensive, coordinated care. Key Responsibilities Deliver Counseling :
Lead one-on-one and group sessions to patients, with group IOP content following the matrix model. Facilitate discussions on recovery related topics, including substance use, recovery, coping strategies, relapse prevention, and daily living skills tailored to early recovery stages. Assist in
Care Transitions :
Support patients transitioning from hospital discharge by helping them navigate the SNF environment, set realistic goals, and connect with resources for ongoing recovery, such as 12-step programs or community support groups. Collaborate with
Clinical Teams :
Work closely with nurses, physicians, therapists, and social workers to integrate peer perspectives into individualized care plans. Participate in case conferences and contribute insights on patient progress and needs.
Facilitate Group Activities :
Lead or co-lead support groups, workshops, and recreational activities focused on building peer connections, emotional regulation, and life skills in a safe, encouraging space.
Monitor and Document Progress :
Observe patient engagement, provide feedback on recovery milestones, and maintain accurate, confidential records in compliance with HIPAA and facility protocols. Promote a
Positive Environment :
Model healthy behaviors, encourage mutual respect among patients, and contribute to a culture of empathy and accountability within the facility.
Community Outreach and Education :
Assist in educating family members or visitors on addiction recovery and occasionally participate in outreach efforts to connect with local recovery networks.
Required Qualifications Licensure:
Current, active Licensed Substance Abuse Associate (LSAA) or Licensed Mental Health Counselor (LMHC) credential in the state of New Mexico.
Experience:
Minimum of 1 year experience providing group therapy or counseling services, preferably in addiction treatment, behavioral health, or a related setting.
Certifications:
Current CPR/First Aid certification required. Must pass background check and drug screening. Preferred Qualifications Certified Peer Support Worker (CPSW): Current CPSW certification with lived experience in recovery is highly preferred.
Additional Training:
Training in trauma-informed care, motivational interviewing, or crisis intervention.
Experience:
Experience in skilled nursing facilities (SNF) or post-acute care environments.
