Work Shifts 10:00 A.M - 6:00 P.M (35 Hours per week) No weekends required Duties & Responsibilities About Bridge to Home Bridge to Home (BTH) is a new innovative program developed by NYC Health + Hospitals that will provide treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) who are ready for discharge from the hospital but do not have immediate access to stable housing. In this voluntary program, BTH will offer its guests onsite ambulatory behavioral health treatment and comprehensive support in a home-like environment. The program aims to keep individuals discharged from the hospital on a path towards sustained success, with the goal of reducing emergency room visits and inpatient hospitalizations, decreasing street homelessness and reliance on shelters, and ultimately moving into permanent housing. Program guests will stay at the home-like environment until they can be transitioned to permanent supportive housing and treatment for a period of up to 12 months. The Coordinating Manager at BTH works in close collaboration with the BTH care team and supports program operations. Reporting to the Site Director the Administrative Director will: Leading the daily ongoing administrative an operational need for Bridge to Home including guest registration, appointment scheduling, and discharge. Support all aspects of the project lifecycle including planning, scope definition, design, execution and delivery, including development of administrative and operational processes, templates, procedures/protocols and other core documents. Working creatively and analytically in a problem-solving environment demonstrating teamwork, innovation and excellence. Support management of day-to-day operations and vender management relationships. Support finance team in reporting, documentation and invoice review and operational approval. Providing status reporting regarding project milestones, deliverable, dependencies, risks and issues, communicating across leadership. Other related duties, as assigned Minimum Qualifications 1. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and 2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity.
