Job Description

The Behavioral Health Consultant functions as a core member of the collaborative care team that involves the patient's primary care provider and other community providers of mental health care, addiction treatment, and alternative health care. The Behavioral Health Consultant is responsible for assisting the primary care provider in facilitating behavioral changes in patients that improves physical, emotional and mental health. Communicate therapeutically with patients. Requires dexterity to accurately type at least 45 words per minute. Job requires prolonged sitting, some bending, stooping and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate office equipment. Requires ability to occasionally lift and carry moderate weight boxes up to 30 pounds. Competent practicing with children and adults of all ages. Assess and treat mental health and behavioral disorders and problems. Assess and treat substance abuse disorders and problems. Coordinate care between medical providers and community resources. Assist medical providers in the development of relevant and effective behavioral health treatment plans. Assist in the implementation of behavioral health treatment plans for identified patients. Current Oregon Qualified Mental Health Professional with no restrictions. LCSW preferred. Previous experience with physician practices preferred. Knowledge of related state and federal regulations regarding mental health and substance abuse. Prefer minimum of 5 years' experience with mental health clinical experience and/or substance abuse treatment. Current American Heart Association Basic Life Support certification. Skilled using Microsoft Office Suite. Base wage + Production Bonus.
