Intensive In-Home Qualified Professional

Job Summary The Intensive In-Home (IIH) service is a team approach designed to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions. This is a time-limited, intensive child and family intervention based on the clinical needs of the beneficiary. The service is intended to accomplish the following: a. reduce presenting psychiatric or substance use disorder symptoms; b. provide first responder intervention to diffuse current crisis; c. ensure linkage to community services and resources; and d. prevent out of home placement for the beneficiary. Responsibilities Maintain a caseload ratio of 1 to 12, ratio shall not exceed Must be available 24 hours per day, 365 days per year, to include crisis intervention Coordinates and oversees the initial and ongoing assessment activities Convene the Child and Family Team for person-centered planning Consult with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations Provide and coordinate behavioral health services and other interventions for the client or other family members with other licensed professionals and Child and Family Team members Monitor and document the status of the client's progress and the effectiveness of the strategies and interventions outlined in the PCP Participate in the person-centered planning process Assist with implement a home-based behavioral support plan with the client and the client's caregivers as indicated in the PCP Provide psychoeducation as indicated in the PCP Assist the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the PCP Assist with crisis interventions Assist the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process. Ensure timely and accurate documentation in accordance with state and agency requirements. Conduct regular reviews of clinical records and service delivery Attends conferences, meetings, audits, and training sessions as requested Comply with Health Insurance Portability and Accountability Act (HIPAA) and adhere to and follow the organizations policies and procedures Accepts other job duties as delegated Requirements Holds a human service degree according to 10A

Minimum 1 year of experience with the children and adolescent population with behavioral needs or a substance use disorder Criminal background check present Not listed on the North Carolina Health Care Abuse Registry Not listed on the exclusion database with the Office of Inspector General Qualified in CPR and First Aid Qualified in Crisis Prevention intervention (CPI) No IIH team member who is actively fulfilling an IIH team role may contribute to the staffing ratio required for another service during that time Knowledge of federal and state laws and regulations Strong computer skills Must demonstrate integrity, sound judgment, demonstrated leadership skills, organizational and strong interpersonal skills.
