Job Description
Job Overview We are seeking a compassionate and proactive Social Worker Assistant to join our dedicated healthcare team. In this vital role, you will support licensed social workers and clinical staff by assisting with patient assessments, data collection, and care coordination across various settings including inpatient facilities, outpatient clinics, hospice, and assisted living environments. Your energetic approach will help ensure patients receive comprehensive support tailored to their individual needs, especially in mental health, behavioral health, and developmental disabilities. Responsibilities Assist with patient intake procedures, including gathering medical histories and documenting relevant information in electronic medical records (EMR systems) Support social workers with case management tasks such as coordinating services for patients with complex needs, including dementia care, autism spectrum disorders, PTSD, and Alzheimer's care Contribute to behavioral health programs by participating in group therapy sessions and behavioral therapy interventions like cognitive behavioral therapy (CBT) and motivational interviewing Collect and organize data for clinical research projects related to mental health, addiction counseling, or post-acute care outcomes Aid in patient assessment processes by observing and recording behavioral changes or symptoms during clinical counseling sessions Maintain strict confidentiality of medical documentation in compliance with HIPAA regulations and ensure accurate medical records management Support crisis intervention efforts by assisting with emergency response procedures and providing compassionate support during high-stress situations Experience Prior experience working in hospital settings, nursing homes, outpatient clinics, or home & community care environments is highly desirable Demonstrated knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for billing purposes, and EMR systems usage Experience supporting patients with developmental disabilities or working with children and seniors in assisted living or geriatrics settings Familiarity with crisis management techniques and working effectively within multidisciplinary teams in inpatient or outpatient contexts Background in clinical research or experience with diagnostic evaluation tools such as ICD coding is a plus Strong skills in data collection, medical documentation, and conducting intake assessments to facilitate comprehensive case management Passion for mental health counseling, behavioral health services, or special education environments Join us to be part of a dynamic team committed to delivering exceptional patient-centered care.
