The MHH CCS staff will work closely with the consumer and consumer's Care Coordinator to improve medication management skills, physical health monitoring, individual skills development, and enhancement, and/or wellness recovery management. The MHH CCS staff will provide training, education, and support to assist the consumer in working towards his or her goals. Rehab Specialist should have personal or professional experience with mental health, substances abuse, mentoring, community resources advocate.
