ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES
Patient Care Interact daily with patients, observing and monitoring behavior and responses to treatment. Charts and reports relevant information to the RN and/or other treatment team members. Upon delegation, obtains patients' vital signs per provider orders, documents findings, and report any changes or abnormalities to the RN. Accompany the licensed medical provider (physician, nurse practitioner, psychiatrist) and/or RN during patient physical exams. Complete documentation packets for all new patient admissions and discharges. Attend shift change reports and participate in care planning discussions. Under RN direction, administer non-medicinal treatments such as foot soaks, cold packs, and topical treatments. Report signs and symptoms of patient violence and/or aggression toward self or others to the RN. Under Provider and/or RN direction, implements seclusion and/or restraint protocols for patients in imminent danger to themselves or others. When necessary, follow one-to-one monitoring protocols for patients in seclusion, restraints, or under suicide or elopement precautions. Assist patients with activities of daily living, including personal hygiene, dressing, laundry, dental care, and making beds. Participate in recreational activities such as community walks, picnics, and games. Document patient intake and output as directed, using flow sheets and reporting abnormalities to the RN. Record and monitor patient weight per provider instructions and report abnormal changes. Under RN direction, conduct non-body cavity searches for contraband, documenting findings appropriately. Escort patients to off-unit activities, including medical appointments, court hearings, community outings, and transition visits. Perform other patient care duties as assigned by the RN. Documentation Chart patient behavior and responses to treatment, including specific incidents, in progress notes. Maintain patient information on required flow sheets, such as 1:1 monitoring, suicide precautions, sleep tracking, and seclusion/restraint records. Complete the Patient Inventory Sheet to document personal belongings at admission and as needed. Other Duties Conduct regular 15-minute safety checks as assigned. Conduct periodic unit checks for safety hazards, including windows, locked doors, and smoke detectors, and reports findings to the RN. Assist the treatment team in patient transition planning and provide emotional support during the process. Assist in training new employees by monitoring interactions with patients and providing feedback. Maintain cleanliness in accordance with Infection Control Policy, including cleaning bathrooms, showers, hallways, and the unit kitchen. Stock unit supplies such as towels, gloves, food items, paper goods, and hygiene products. Promote a safe unit environment by always remaining vigilant.

Communicate patient concerns and irregularities to the charge nurse promptly.
Required:
High School Diploma or GED Valid driver's license Successful completion of background check AVADE training and CPR certification within 90 days of hire
Preferred:
2-3 years of experience in behavioral health setting Certified Nursing Assistant
