Essential Duties:
Facilitate psychoeducational therapeutic groups that are aligned with evidence-based practices Help clients (1:1) to work toward specific goals (Treatment Plan) and document progress that is made towards those goals at least quarterly Facilitate effective communication with interdisciplinary team (IDT) regarding client care Encourage linkage between client and community agencies Advocate for client rights and well-being Accompany clients on appointments, as instructed by Clinical Director Support nursing staff as needed when clients are non-adherent to the care being provided Work in tandem with nursing staff to promote client group attendance, Activities of Daily Living (ADLs), medication adherence, appropriate behavior, and progress toward treatment plan goals, etc Do we still have your interest?
