Provides quality care that is personal to the patient's needs under the supervision of the interdisciplinary team to support individual diagnosis and interventions. Will assist with documenting necessary data to implement a successful plan of care for each patient. Will serve as a resource for socially complex cases. #STC Education Requirements 4 year Bachelor's Degree
