  • Part-time, Full-time, Contract Nexus Health Innovations is a growing culturally competent mental health agency servicing adults with severe and persistent mental illness (SPMI) within the 4 counties of the metro (Hennepin, Ramsey, Dakota, and Anoka) as well as counties in Central Minnesota.

We are offering a full-time mental health practitioner position for in-home based services for adults with SPMI. The purpose of the Adult Rehabilitative Mental Health Services (ARMHS) mental health practitioner is to provide rehabilitative services to clients in-home and/or in the community with mental illness, in the form of instruction, support, and skill training for the purpose of promoting basic living and social skills, community intervention, and transitioning to community living. Qualification This position requires one of the following three requirements: Holds a high school diploma/Associates degree
AND 6,000
hours of supervised experience in the delivery of services to persons with mental illness BA/BS degree from an accredited college or university AND has at least 2,000 hours of supervised experience in the delivery of services to persons with mental illness Holds a Master's or other graduate degree in one of the behavioral sciences or related fields from an accredited college or university Responsibilities Provide quality patient care to clients/families with mental health diagnosis based on the individual treatment plan Assist with collecting assessment data through interviews, 1:1 interactions Create, review and update treatment plans and functional assessments in a timely manner (depending on qualifications) Participate in regular staff meetings/trainings Responsible for documenting session interventions and billable time into our electronic health record (EHR) system within 24 hours of service date Create work schedule directly with assigned client and provide services in their home and in the community Observes physical, psychological and safety status of clients on an ongoing basis Report changes, noted by self or reported by client Must have a valid driver's license, acceptable driving record, and reliable vehicle.
