In this role, you will deliver structured, goal-oriented interventions that focus on improving daily living skills, medication adherence, problem-solving, and community integration. Services are person-centered and designed to increase independence, stability, and overall functioning. You will collaborate with the treatment team to implement individualized service plans while supporting each individual's progress toward recovery and self-sufficiency. This is a paid opportunity to make a meaningful impact while working in a supportive, team-driven environment. Duties Deliver Mental Health Skill-Building Services (MHSS) in the individual's home and community in accordance with DMAS and DBHDS requirements Implement individualized service plans by teaching and reinforcing daily living skills such as hygiene, meal preparation, budgeting, and household management Support medication adherence by providing education, reminders, and coordination with prescribing providers Utilize evidence-based and person-centered interventions to improve coping skills, emotional regulation, and problem-solving abilities Assist individuals with accessing community resources, including medical, social, housing, and vocational supports Monitor and document progress toward individualized goals during each service session Complete accurate, timely, and audit-compliant documentation, including progress notes and service summaries Collaborate with case managers, therapists, medical providers, and natural supports to ensure continuity of care Provide crisis support and follow established protocols when individuals experience psychiatric or behavioral instability Maintain professional boundaries while building rapport and therapeutic relationships with individuals served Participate in required supervision, team meetings, and ongoing training in compliance with agency and regulatory standards Ensure services are delivered in accordance with Medicaid guidelines, agency policies, and ethical standards Promote independence by gradually reducing support as individuals demonstrate increased functioning and stability Qualifications Must meet criteria for Qualified Mental Health Professional (QMHP), Qualified Paraprofessional in Mental Health (QPPMH), or QMHP-Trainee (QMHP-T) in accordance with Virginia DBHDS standards Minimum of one (1) year of experience providing community-based services to individuals with serious mental illness or related behavioral health needs Bachelor's degree in a human services field (or equivalent experience as defined by DBHDS for
QPPMH/QMHP-T
roles) Demonstrated knowledge of serious mental illness, psychiatric symptoms, and recovery-oriented care Experience delivering skill-building, case management, or psychosocial rehabilitation services preferred Ability to implement person-centered, goal-oriented interventions in home and community settings Strong communication and interpersonal skills with the ability to engage individuals from diverse backgrounds Ability to work independently while maintaining collaboration with a multidisciplinary treatment team Knowledge of Medicaid documentation requirements and ability to complete accurate, timely, and audit-compliant notes Valid driver's license, reliable transportation, and ability to travel within the community to provide services Current CPR/First Aid certification (or ability to obtain within required timeframe) Ability to pass background checks, including criminal history and child/adult abuse registry screenings Basic computer skills for documentation, communication, and use of electronic health records Commitment to maintaining professional boundaries, ethics, and confidentiality in all interactions
