The Certified Nurse-Midwife (CNM) provides comprehensive, evidence-based outpatient women's health and gynecologic care to patients across the lifespan within a Caring Health Center setting. This role emphasizes preventive care, reproductive health, and management of common gynecologic and hormonal conditions. This position does not include intrapartum or delivery services. The CNM may perform initial prenatal evaluation during the first trimester and is responsible for coordinating a timely, structured transition of care to an affiliated obstetric provider for ongoing prenatal management and delivery. The CNM practices independently within the full scope authorized under Massachusetts General Laws and Massachusetts Board of Registration in Nursing regulations, and functions collaboratively within an interprofessional, team-based care model consistent with Caring Health Center's mission of advancing health equity for underserved populations. All care is delivered through a trauma-informed lens, recognizing the impact of adverse experiences on health outcomes and centering patient safety, dignity, choice, and empowerment in every clinical encounter. Master's or Doctoral degree from an ACNM-accredited nurse-midwifery program Current certification by the American Midwifery Certification Board (AMCB) Active Massachusetts RN and APRN/CNM
licensure, or eligible and able to obtain prior to start Active DEA registration and Massachusetts Controlled Substance Registration (CSR) with full prescriptive authority Current BLS/CPR certification; ACLS preferred Principle Responsibilities 1. Contraceptive Management and Family Planning Provide comprehensive, patient-centered contraceptive counseling encompassing all FDA-approved methods, including risks, benefits, alternatives, and patient values. Prescribe and manage all forms of hormonal and non-hormonal contraception (combined oral contraceptives, progestin-only pills, patch, ring, injectable, emergency contraception). Perform insertion and removal of long-acting reversible contraception (LARC), including intrauterine devices (hormonal and copper IUDs) and subdermal contraceptive implants. Conduct pre-procedure counseling, obtain informed consent, and perform - procedure follow-up. Troubleshoot and manage LARC-related complications within scope of practice. 2. Preventive and Routine Women's Health Care Perform comprehensive well-woman examinations and periodic health assessments across the lifespan. Conduct cervical cancer screening (Pap smear and HPV co-testing) per current ACOG and USPSTF guidelines. Perform clinical breast examinations; evaluate breast complaints; order and follow up on mammography and ultrasound per evidence-based guidelines; facilitate timely referral for suspicious findings Provide age-appropriate preventive counseling on health maintenance, immunizations, nutrition, and lifestyle 3. Gynecologic Evaluation and Management Diagnose and manage common gynecologic conditions, including: Vulvovaginal disorders: bacterial vaginosis, vulvovaginal candidiasis, trichomoniasis, vaginitis Vaginismus and sexual health concerns Dysmenorrhea (primary and secondary) and pelvic pain Abnormal/dysfunctional uterine bleeding (AUB/DUB) Polycystic ovary syndrome (PCOS): diagnosis, metabolic counseling, cycle regulation, fertility awareness Menopause and perimenopause: vasomotor symptom management, genitourinary syndrome of menopause (GSM) Hormone replacement therapy (HRT): individualized systemic and local regimens; monitoring and adjustment Order, interpret, and act on relevant laboratory and diagnostic studies Coordinate with gynecology or other specialists for surgical evaluation or complex management as indicated 4. Sexual and Reproductive Health Screen, diagnose, and treat sexually transmitted infections (STIs) per CDC and Massachusetts DPH guidelines (chlamydia, gonorrhea, syphilis, HIV, hepatitis B & C, trichomonas) Provide partner notification counseling; connect patients with public health resources Provide reproductive health counseling, fertility awareness education, and preconception care 5. Prenatal Care and Care Transitions May perform initial prenatal evaluation during the first trimester and provide early pregnancy counseling. May initiate and coordinate appropriate prenatal laboratory and diagnostic workup. Execute structured, documented handoff to external obstetric providers for continued prenatal care and delivery. Ensure timely communication with receiving providers and accurate documentation through transitions of care. 6. Care Coordination and Team-Based Care Collaborate with primary care, behavioral health, nursing, medical assistants, social work, and care management staff. Participate in multidisciplinary case discussions, care planning meetings, and population health programs. Address social determinants of health impacting patient outcomes; connect patients with enabling services. Support panel management and proactive outreach for preventive care gaps. 7. Documentation, Compliance, and Quality Complete accurate, timely clinical documentation in the electronic health record (EPIC) consistent with Caring Health Center UDS reporting requirements. Adhere to all Caring Health Center regulatory requirements, clinical protocols, and organizational policies including HIPAA. Participate in quality improvement initiatives, clinical audits, and performance benchmarking Meet established productivity, access, and quality metrics as defined by leadership Preferred Qualifications Prior clinical experience in an FQHC, community health center, or safety-net setting Proficiency with EPIC or comparable EHR system Bilingual/bicultural ability (Spanish, Russian, Arabic, Portuguese, or other language reflective of the patient population) Demonstrated experience providing care to underserved, multilingual, or culturally diverse populations Training or experience in trauma-informed care and reproductive justice frameworks Work Environment and Schedule 100% outpatient clinical role; no inpatient or delivery responsibilities Standard clinic hours aligned with organizational scheduling model Participation in site-based or cross-site coverage as operationally required Performance Expectations Deliver high-quality, evidence-based care consistent with organizational and national clinical standards Maintain patient access and panel management expectations as defined by leadership.
