Job Description

Must be licensed in Ohio as LSW, LPC or higher Summary/Objective Works with individuals, groups and communities to improve mental health and behavior. Provides therapy to clients predominantly in the school environment as well as in the home or community settings as deemed necessary. Creates and updates diagnostic assessments, treatment plans and treatment plan reviews. This role will work specifically with clients located at the LEAP locations in Toledo and Wauseon, OH. We are open to a split caseload between both, or Part-time availability for one of the two locations. Essential Duties and Responsibilities
  • Deliver mental health services to clients in-school or in-home as deemed necessary. Services predominantly include going into the school.
  • Maintain set schedule consistent with both the needs of the client and the school/s serviced
  • Provide therapeutic interventions to the identified client in order to enhance and foster the client's optimal functioning and interactions in the home and community.
  • To continually assess client needs, service availability and appropriateness.
  • Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person Centered Plan.
  • Provide appropriate documentation for service delivery and submit said documentation as specified by Medicaid and CARF standards.
  • Maintain required direct care interaction including all necessary documenting and reporting, attendance of meetings, trainings, and all standard measurable metrics and agency standards set for role
  • Maintain Client Interaction of 65% of worked hours
  • Assist in creating and maintaining group therapy sessions
  • Complete and maintain all continuing education and licensure
  • Maintains quality results by following agency standards-attend clinical supervision, manage caseload effectively, manage and maintain treatment plans, adhere to on time and quality documentation standards
  • Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.
  • Serves and protects the agency by adhering to professional standards, agency policies and procedures, federal, state, and local requirements, and CARF standards.
  • Perform other duties, as assigned, to the need of the agency deemed necessary and appropriate (including but not limited to completion of Social Security forms, authorization or billing forms, phone calls, and participating in case consultation meetings with schools). Measurable Metrics

  • Licensed Social Worker or Counselor

  • Excellent time management, organization, attention to detail, quality focus, professionalism, productivity, thoroughness, coordination, deadline-oriented, internal communications, reporting skills.
