The Nurse Practitioner will be a primary on site provider of care to hematology and oncology patients, high risk cancer patients, as well as patients using the services of the infusion center, working in collaboration with CCIC team member including: Hematology/Oncology Specialist Physician, CCIC Team Lead Provider, Oncology Nurse Navigators, CMAs, RN Manager, Pharmacists, Infusion RNs and CNAs. The Physician Assistant will work collaboratively to support all patients of the CCIC with supportive care visits, surveillance visits, symptom assessment and management, and oncology/infusion acute care needs, including infusion reaction response. The Physician Assistant will also provide clinical support to ONNs, CMAs, Infusion RNs, and CNAs when needed. The Physician Assistant will work diligently to support the care plans initiated by Hematology/Oncology physicians in efforts to ensure patients are receiving high quality coordinated care. The Nurse Practitioner will also work closely with PCPs to ensure seamless, concerted efforts to support patient s health care goals. Primary job duties will be located in a single site, the Cancer Care and Infusion Center (CCIC) a part of the Chief Andrew Isaac Health Center operated by the Tanana Chiefs Conference (TCC) a consortium of Interior Alaska Native tribes. Under the direct supervision of CCIC Team Lead Provider, the Nurse Practitioner will: 1. Perform comprehensive appraisals of patients' health status by taking health histories and performing physical examinations. 2. Provide care to patients including medical management of acute oncological and hematological illness and emergencies in the CCIC. 3. Work closely with the Hematology/Oncology (Hem/Onc) Specialist Physician, other CCIC providers, pharmacists, and nursing staff in patient care decisions. 4. Participate in follow up and surveillance of post cancer care. This will include ensuring that surveillance follows current National Comprehensive Cancer Network (NCCN) guidance as well as the recommendations of the consultant Hematologist/Oncologist. 5. Collaborate in care with and referrals to specialty providers (e.g. ANMC Hem/Onc, Radiation Oncology, General Surgery etc.). 6. Actively engage in the assessment and evaluation of patients undergoing treatment, including physical examinations including preoperative exams (and/or referral to the patient s PCP for those exams), ordering and interpreting labs and imaging, and ordering medications. 7. Manage side effects and complications of chemotherapy/immunotherapy and other treatments. 8. Be available for consultation by medical staff regarding patient care. 9. Work with medical providers to coordinate and supervise infusions of medication not related to oncology (e.g. IV fluids, iron transfusion, biologics, endocrine and rheumatologic agents, antibiotics, etc.). 10. Provide supportive care visits as needed to patients that are admitted at local hospital or local SNF in collaboration with their lead care teams (hospitalist, palliative care provider, RN). 11. Provide palliative care and remote end of life support to CCIC patients as needed. 12. Document discussion of the medical care plan (hematology and/or oncology and others) with the patients. 13. Participate in education of patients and families. 14. Maintain timely and accurate medical records. 15. Recognize and take corrective action for abnormal patient responses to medications. 16. Participate in the development of policies and procedures governing the functioning of the Hematology/Oncology Section and the Cancer Care and Infusion Center. 17. Serve on relevant committees and participate in quality improvement projects. 18. Obtain awareness and ensure compliance with all IHS and AAAHC standards associated with the provision of medical services. 19. May be required to do home visits.Other Responsibilities:
1. Perform other job duties as assigned.Minimum Qualifications Minimum Qualifications:
1. Must be a licensed APP medical provider (PA-C, NP). 2. Must be able to meet criteria for appointment to the medical staff and be in good standing. 3. Must hold a valid license to practice in State of Alaska. 4. Three (3) years minimum primary care experience, five (5) years preferred. Additional experience in cross-cultural health care is strongly preferred. 5. Experience in Hematology/Oncology is not required but ongoing CME choices should reflect this specialty role. 6. Current BLS and ACLS certification. 7. Current DEA registration (or eligibility to obtain) certificate for prescribing authority. 8. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirements and State of Alaska background check.Knowledge, Skills and Abilities :
1. Capacity for making quick, intelligent decisions in a high risk medically complex patient population. 2. Proven skills in patient triage and workload prioritization. 3. Ability to work with consultants and staff in a collaborative manner. 4. Strong verbal, written, and interpersonal skills. 5. Experience and history handling stress and working under pressure. 6. Ability to provide highly compassionate and culturally sensitive care to a vulnerable population. 7. Desire to provide concierge type, patient-centered, wrap-around, uniquely tailored care.Supervision:
No supervisory responsibilities are anticipated.Supervision Physical demands:
Work is primarily at Chief Andrew Isaac Health Center. Position requires recurring bending, stooping, kneeling, stretching, and reaching. No Lifting >20kg is anticipated.Hazardous Environment:
Although risk is very low, there is a possibility of exposure to hazardous drugs (HDs) and chemicals in the workplace. Employee will maintain compliance with job-specific protocols, competencies and trainings to reduce risk to self, other staff, and patients.Summation Summary:
