Job Description

Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary care services, including health assessments, diagnosis, treatment, and patient education across diverse populations. Your expertise will support patients of all ages, from pediatrics to geriatrics, ensuring they receive high-quality, accessible healthcare. This position offers an exciting opportunity to make a meaningful impact on community health through innovative clinical practices and patient-centered care. Duties Conduct thorough physical examinations and health assessments for patients across various age groups, utilizing advanced clinical evaluation techniques. Diagnose and manage acute and chronic illnesses, employing evidence-based treatment plans tailored to individual patient needs. Administer medications, including injections and IV infusions, following proper protocols and safety standards. Perform specialized procedures such as suturing, dermal fillers, spinal taps, and esthetic laser treatments when appropriate. Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, and Cerner to document patient encounters accurately and efficiently. Coordinate care with specialists in gastroenterology, cardiology, infectious disease care, behavioral health, and other disciplines to optimize patient outcomes. Engage in telehealth consultations to expand access to care while maintaining compliance with HIPAA regulations. Participate in clinical research initiatives and quality improvement projects to advance evidence-based practice standards. Provide patient education on topics such as family planning, nutrition, memory care, hospice & palliative medicine, and senior care. Support discharge planning, case management, utilization review, and working with individuals with disabilities or developmental disabilities. Requirements Valid Family Nurse Practitioner license with current certification from the American Association of Nurse Practitioners (AANP) or equivalent. Proven experience in primary care settings with proficiency in medical office operations and EMR/EHR systems such as Epic or Athenahealth. Demonstrated expertise in procedures including suturing, phlebotomy, catheterization, venipuncture, sterile processing techniques, and basic life support (BLS). Experience working with diverse populations including pediatrics (including neonatal ventilator management), geriatrics (memory care), hospice & palliative medicine patients, and individuals with developmental disabilities. Critical care experience in ICU or Level I/II trauma center environments is highly desirable. Knowledge of CPT coding and ICD coding systems (ICD-9/10) for accurate documentation and billing purposes. Strong understanding of infection control practices and aseptic techniques across various clinical procedures. Excellent communication skills with the ability to triage effectively during urgent or emergent situations in outpatient or inpatient settings. Ability to collaborate seamlessly within multidisciplinary teams across hospital medicine, emergency medicine, occupational health, home care services, and outpatient clinics.
