Job Description

Job Summary We are seeking an experienced and patient-focused Nurse Practitioner (NP) to join our healthcare team. This position is responsible for advanced patient assessment, clinical triage, care coordination, and chronic disease management in an onsite clinical setting. The ideal candidate will be comfortable making clinical decisions, managing patient escalations, coordinating follow-up care, and determining appropriate levels of intervention based on established protocols and evidence-based practice. This role emphasizes clinical judgment, patient management, and interdisciplinary collaboration rather than routine bedside nursing tasks. Responsibilities Perform comprehensive patient assessments and clinical evaluations. Triage patient concerns and determine appropriate levels of care. Identify and manage clinical escalations, including cases requiring urgent provider intervention or Emergency Room referral. Provide patient education regarding treatment plans, medications, follow-up care, and preventive health measures. Coordinate care among physicians, specialists, hospitals, and other healthcare providers. Monitor and manage patients with chronic health conditions. Review patient records, laboratory results, and clinical documentation to support care decisions. Ensure timely follow-up and continuity of care. Collaborate with physicians and interdisciplinary healthcare teams to optimize patient outcomes. Maintain accurate and compliant clinical documentation.
