Job Description

Job Overview We are seeking a dedicated and highly skilled Board-Certified Family Nurse Practitioner to join our dynamic healthcare team. In this vital role, you will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. Your expertise will support patient-centered care through thorough assessments, diagnosis, treatment planning, and health education. The ideal candidate will bring a passion for patient advocacy and possess a broad skill set that spans acute and chronic care management, with experience in various clinical settings. Responsibilities Conduct detailed patient assessments including vital signs, physical examinations, and medical history reviews to determine health status and needs. Diagnose and treat acute illnesses and chronic conditions across a wide age range, utilizing evidence-based practices to optimize patient outcomes. Manage medication administration, including injections, IV infusions, suturing, and dermal filler procedures while adhering to sterile techniques and safety protocols. Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks for accurate documentation of patient encounters, treatment plans, and medical records. Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to ensure comprehensive patient care. Provide health coaching and education tailored to individual needs, including family planning, nutrition counseling, memory care strategies, and geriatric health maintenance. Participate in clinical research initiatives or quality improvement projects aimed at advancing clinical practices within the organization. Skills Extensive experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth for efficient documentation and patient management. Proficiency in performing procedures like suturing, catheterization, spinal taps, phlebotomy, and dermal fillers with aseptic technique. Strong knowledge of pharmacology related to psychopharmacology, pain management, infectious disease care, hospice & palliative medicine, and acute pain management. Ability to perform diagnostic evaluations using laboratory experience including specimen collection/processing and medical imaging interpretation. Skilled in managing diverse clinical scenarios such as ICU or Level I Trauma Center cases, emergency medicine protocols, telehealth consultations, occupational health issues, and working with individuals with developmental disabilities or dementia care needs. Familiarity with CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-10 standards for accurate billing and documentation. Critical care experience including ICU or PICU settings; hospital medicine; emergency room experience; working within a Level II trauma center environment. Knowledge of HIPAA regulations ensuring patient confidentiality; experience with medical documentation standards; proficiency in physical examinations and vital signs assessment. Capable of managing complex cases involving gastrology procedures like gastric lavage or tube feeding; cardiology assessments such as cardiac catheterization; airway management techniques; ventilator support; and infection control protocols.
